Your visit to Harmony Garden Massage Therapy is designed to be a most beneficial, positive and relaxing experience.  Please find provided a list of definitions, policies and explanations to foster the most comfortable and therapeutic environment for wellness.
 
DEFINITIONS
 
Client – The person receiving massage, bodywork, and/or spa treatments.
LMT – Licensed Massage Therapist; the person providing the service for the massage, bodywork and/or spa treatment sessions and who has completed and passed the state education qualifications and maintains a massage therapy license along with continuing education requirements.
Session – The length of time booked for massage therapy, bodywork and/or spa treatments. 
 
EXPECTATIONS
 
1.) CLIENT will receive the massage, bodywork and/or spa treatment  session for the time scheduled in a professional, safe, clean, nurturing environment. 
2.) New CLIENTS should allow approximately an extra 15 minutes with their first session for consultation.
3.) CLIENT is responsible for payment in full at the time of their scheduled session. 
4.) CLIENTS must be 18yrs. of age or older to receive massage, bodywork and/or spa treatment sessions.  Minors must have signature of parent or legal guardian who must be present in the treatment room at all times.
5.) Client will be draped at all times except for the portion of the body being worked on. 
6.) LMT does not provide breast massage of female clients without the written consent of the client and/or their physician.
7.) Client may terminate the session at any time and may still owe part or the full session fee. 
 
LATE AND CANCELLATION POLICIES
 
1.) There is no charge for cancellations made at least 24 HOURS in advance. 
2.) If CLIENT is running late for the session, the LMT will begin a session no more than 15 minutes into the scheduled appointment time.  The LMT will provide session services for the remainder of the session time and CLIENT is responsible for full payment of the session fee.    
3.) NO CALL/NO SHOW – CLIENT is responsible for full payment of the missed session.  No new sessions will be booked until payment has been made.  Failure to pay may result in no future appointments being booked and/or legal action.
4.) LMT reserves the right to deny service to anyone at any time for any reason.  Client may still owe part or the full session fee.
 
PAYMENT
 
Accepts cash, checks*, and credit cards.
 
*No temporary checks; CLIENT must be able to provide DL# and phone number.  There is a $30.00 returned check fee.  Failure to pay in full for the session and any other related fees may result in no future appointments being booked and/or legal action.
 
IMPORTANT INFORMATION

The massage therapist does not engage in breast massage of female clients without the written consent of the client and/or their physician.  The massage therapist will use appropriate draping throughout the session unless otherwise agreed to by both the client and the massage therapist.  Clients must be 18yrs. of age or older.  Minors must have signature of parent or legal guardian who must be present in the treatment room at all times. 
 
Massage Therapy is not a substitute for medical examination and/or diagnosis.  It is advised that you see a physician for any physical ailment you might have. Client understands that massage therapists do not diagnose or treat illness, disease or any other physical or mental disorder. Massage therapists do not prescribe medical treatment or pharmaceuticals, nor does he/she perform any spinal thrust adjustments.  Client understands that nothing said in the course of the session by the massage therapist should be construed as diagnoses or treatment.  Client affirms that they have stated all known medical conditions and answered all questions honestly.  Client agrees to keep the practitioner updated as to any changes in medical profile and understands there shall be no liability on the practitioner’s part should Client fail to do so.
 
If uncomfortable for any reason, either the massage therapist or the client may end the session.  Client may still be liable for the full or partial payment of the scheduled appointment.  Client understands that any remarks or gestures made by Client and interpreted by the massage therapist as sexually suggestive will result in the immediate termination of the session and Client will be liable for full payment of the scheduled appointment.
 
CONTRAINDICATIONS

Contraindication – A condition that makes a particular treatment improper or undesirable.
Local/Regional – DO massage but avoid a particular area.
General - Avoidance of application.  DO NOT MASSAGE.
 
Please make sure your massage therapist is made aware of any and all medical conditions that may or do contraindicate massage.  Examples of contraindications include but are not limited to:
 
Sneezing, sore throat, runny nose, fever, heart conditions, severe hypertension, blood clots, fractures or recent surgeries (within the past six weeks), and allergies. 
 
Females should advise if they are pregnant or if there is ANY possibility that you may be pregnant.
 
Some medications may contraindicate massage.  Advise your massage therapist of any and all prescription and/or OTC (over-the-counter) medications you take or have taken prior to your session.
